Katniss knew he was being controlled but had to defend herself. The fight forced her to confront the damage done to Peeta while still trying to save him. It ended with Peeta starting to fight off the brainwashing but left him emotionally damaged. The fight highlighted the personal cost of war and how trauma affected both of them. It also added weight to their relationship and made their eventual reunion feel more earned.
