The Map That Leads to You is about Heather (played by Madelyn Cline), who goes to Europe with her friends after completing her graduation. She is enjoying her vacation when she meets a guy named Jack (KJ Apa). As they start getting to know each other, love slowly blossoms.

As The Map That Leads to You moves towards the ending, we see Heather return to New York and start working in her new job. But she can't stop thinking about Jack and is figuring out his reason to ghost her. She receives a letter from Raef that Jack had given him. It mentioned that Jack's illness came back, and he doesn't want her to see him die.

The ending of the diary references a festival that happens in Santa Pau. Since Jack had told her about it, she went there to find him. The two had spent two weeks in Europe and helped each other with certain things. She taught him to see the positive side of the world, while Jack made her realize that it's important to be in the moment.

The two fall in love, and Heather doesn't want to give up on their relationship. However, Jack's circumstances, particularly the fact that he does not have much time left to live, force him to leave her for her own benefit.
