Why General Hospital's Drew Cain is NOT Dawn of Day's Shiloh

Puzzled General Hospital fans want to know what the heck happened to good guy Drew Cain. After he was released from prison in 2023, it seemed he had undergone a personality change. In mid-2025, he bears almost no resemblance at all to his former self and seems like a brand-new character, but why?

The Shiloh theory

Some fans have speculated that Drew (Cameron Mathison) is not Drew at all, but really Shiloh (Coby Ryan McLaughlin), aka David Henry Archer, the charismatic and sociopathic cult leader who had Willow (Katelyn MacMullen) under his thumb and fathered her first child, the late Jonah.

Drew and Shiloh knew one another once upon a time in Afghanistan when Drew was a Navy SEAL and Shiloh was a mercenary. While Sam (Kelly Monaco) allegedly killed Shiloh in 2019 using a flare gun, we never actually saw the body after it fell from the Haunted Star, and only heard it had been identified.

While Shiloh might definitely be alive and could have kidnapped the real Drew and traded places with him, there are some things about the current version of Drew that make us believe he is not Shiloh at all, but instead, is actually Drew.

Shiloh did not care one bit about the Quartermaines and certainly would never be obsessed with having their last name. He also wouldn't care what Monica thought of him. When Drew learned that Monica had barred him from the Quartermaine grounds, the look of hurt on his face was palpable. Since nobody else was around to see it, that told us the look was real.

So, if Drew is nothing like the Drew we knew before and not Shiloh, as Shiloh would never care about being a Q, then who is he? Well, maybe he is actually Drew Cain, but the real version of the character. We saw some hints of the "real" Drew when Franco (Roger Howarth) had Drew's memories. Franco's version was kinda a smarmy guy, but not as smarmy as he is now.

Could the beating in the shower have morphed Drew into the man he was before, but a more intense version? Perhaps Drew had a mental breakdown during those moments, wondering why the heck he threw himself on his sword for Carly just to end up beaten in a prison shower.

Whatever caused Drew's change, we do believe he is indeed Drew, but we also believe someone needs to try to kill him already. So, what are we waiting for? Let's accept that Drew is Drew and get a murder or attempted murder mystery going. Be a proper soap, GH, and give us the 'Who shot Drew?' story we've been craving.
