Peacock's thriller mystery All Her Fault delivered everything it promised and more. An adaptation of Andrea Mara's 2021 novel of the same name, the show followed the disappearance of a 5-year-old from his playdate, which never really existed. The central mystery that one would assume the show revolves around is who kidnapped the 5-year-old, but this mystery is revealed early on in the Peacock thriller, revealing that a nanny named "Carrie" was the culprit.
The real mystery was her intentions behind the kidnapping. All Her Fault managed to maintain the sense of mystery and surprise until the very last minutes of the show. By the end, however, she suffers one of the most tragic fates in the show; she is killed.

What happened to the nanny in All Her Fault who had kidnapped the 5-year-old?
Chicago businesswoman Marissa Irvine agrees to a playdate for her 5-year-old Milo with a woman named Jenny and her son Jacob over the phone, but she's never met the woman before. However, when she reaches the given address to pick up her kid, she's shocked to discover that Milo isn't there, and the old lady who opens the door has no idea who Jenny or Jacob is.
Who is Carrie in All Her Fault?

As the police start investigating the disappearance, Jenny's nanny Carrie Finch, played by Sophia Lillis, soon becomes the prime suspect behind the disappearance in All Her Fault. It is also revealed at the start that Carrie had created a fake identity just to get a job as Jenny's nanny, so it becomes extremely clear that Carrie is the one who kidnapped Milo, but nobody knows why.
As the drama unfolds in All Her Fault, it is revealed that Carrie abducted Milo because she believes that Milo is her own son and orchestrated the entire scenario to take Milo from the Irvines. The show goes through a lot of twists, turns, and dramatic reveals to actually confirm this and that Carrie is actually Josephine 'Josie' Murphy.
Why did Carrie kidnap Milo in All Her Fault?

It is revealed that years ago, Marissa and her newborn baby had gotten into a car accident with Josie/Carrie and her kid. While Marissa's kid had passed away in the accident, Josie/Carrie's Milo had survived. Marissa's husband, Peter, thinking Carrie/Josie was dead, decided to switch the babies and take Milo home as his own son.
In the present day, Peter attacks Carrie/Josie's dad, Rob, in a ransom meeting unbeknownst to Carrie/Josie and takes Milo back. On finding this out, Carrie/Josie goes back to the Irvines's home with a gun, not to take Milo, but to reveal the entire truth to Marissa, who was unaware that her own child had died in the accident and that her husband had switched the babies.
What happened to Carrie at the end?
However, just as she is about to reveal the incriminating truth with proof to Marissa, Peter tackles her, and in the heat of the moment, her gun is fired, and she is found dead. Peter claims that it was self-defence, but Marissa is more than aware that there's something really big that Peter is trying to hide.
So, even though Carrie/Josie suffers a tragic fate in All Her Fault, she manages to bring Peter's crimes to light and is partially responsible for his fate at the end of the show.
