Josh Charles’s performance as Martin Best as he navigates through the changes that his life has now come across because of his secret leaves an impact. In the first episode, Martin’s behavior, especially the way he tries to avoid having conversations with the townspeople, adds more humor to the storyline. Despite his incessant tries to stay out of the drama of the town, he gets constantly dragged into the middle of these situations.

Abigail Spencer, with projects such as Angela's Eyes, Burning Love, etc., always brings her A-game. This is also the case for this series. Abigail Spencer’s Louisa is a schoolteacher. She is stuck in a complex situation after she refused to continue with her wedding just two weeks before.

For a small town with a close-knit community, as in Port Wenn, staying away from the constant worries of the townspeople is not easy. Hence, everywhere Louisa goes, she gets surrounded by the townspeople. In the first episode, Josh Segarra’s Sheriff Mark Mylow also becomes crucial to the storyline. After interacting with him, Martin decides to give this small town another chance.

Other cast members of Best Medicine are Cree, Annie Potts, Didi Conn, Stephen Spinella, etc. In the first episode of Best Medicine, Martin arrives at Port Wenn with the desire to start over after an incident in his past. Initially, this past circumstance is not revealed; however, slowly, as the storyline progresses, it comes to light.

While discussing about leaving this town, Martin reveals to his aunt Sarah about his past mistake, which made him leave his successful career in Boston and join here. During an operation, he froze midway, causing another doctor to take over the operation.

The patient was just an eight-year-old girl. Hence, it put pressure on his mind, causing him to freeze. Though he blames himself, Sarah tries to make him understand that it was not his fault. After this incident, every time he sees blood, he gets panic attacks. It causes him to even get unconscious.

At the end of the first episode, after getting punched during an altercation, he decides that this town is not for him. Before he is able to go, he speaks with Mark. Mark’s words about being helpful make him rethink his choice to stay back and help the townspeople. For now, Martin has decided to stay back; however, it can be understood that the upcoming episodes will definitely create more trouble for him.
