How did Otis Zvonecek die in Chicago Fire?

In the Chicago Fire Season 7 finale episode titled I'm Not Leaving You, the team arrives at a factory whose basement caught fire. Around 50 people were stuck inside the basement of the mattress factory. The firefighters struggled and rescued some people.

However, when Severide finds there isn't another entrance except for the staircase, which was engulfed with fire, Matt, Severide, and the other teammates used a rope to reach the basement.

Soon, they start the rescue mission, but it's quite chaotic. Things take an ugly turn when they hear the whistle of the boilers. Hermann warns them that if it bursts, it would leave several dead. Boden orders them to evacuate, but Severide doesn't want to leave the victims there, and Matt decides to help him. Mouch also tags along.

On the other hand, Hermann asks Ritter to leave, but he does not and continues to spray water on the malfunctioning boiler. The episode ends with the boiler reaching its peak.

Chicago Fire Season 8 Episode 1 begins with the firefighters trying their best to control the situation, and as they finally convince Ritter and Hermann to leave the boiler room, they run towards the door. The boiler bursts, and the fire almost engulfs Otis, and he slams the door to save his fellow firefighters.

When they rescue Otis and cut through his suit and mask, he's barely alive. As the ambulance rushes towards the Chicago Med with Otis' body, Cruz cries his heart out for his best friend. In the hospital, Cruz delivers one of the most heartwrenching moments; he asks his best friend to keep a space for him in heaven. He says,

"I love you brother. I am gonna miss you."

Otis replies in Russian, which translates to,

"I will be with you always, Brother."

Finally, Otis Zvonecek dies in the hospital, leaving his best friend Cruz shattered.
