Mr. Robot premiered in 2015 and ran for 4 seasons till 2019. The official synopsis of the show, as per Rotten Tomatoes, mentions,

“Young, anti-social computer programmer Elliot works as a cybersecurity engineer during the day, but at night he is a vigilante hacker. He is recruited by the mysterious leader of an underground group of hackers to join their organization. Elliot's task? Help bring down corporate America, including the company he is paid to protect, which presents him with a moral dilemma.”

The synopsis further reads,

“Although he works for a corporation, his personal beliefs make it hard to resist the urge to take down the heads of multinational companies that he believes are running -- and ruining -- the world.”

In Mr. Robot, Elliot is someone who struggles to form genuine connections with others. It was only Shayla Nico who could break down his walls, and they ultimately shared a romantic relationship. Therefore, Shayla’s death, because of Fernando Vera, becomes an extremely impactful incident in Elliot’s life.

Shayla was killed by Fernando Vera in Mr. Robot

Fernando Vera is one of the antagonists in Mr. Robot. His introduction to Elliot happens through Shayla as he supplies her with the drug suboxone, which could not be found elsewhere. As Fernando keeps supplying Shayla with drugs, he wants to take her out on a date, which she refuses.

However, being the psychopath that Fernando is shown to be in Mr. Robot, he drugs Shayla and rapes her. Elliot finds out when he comes to Shayla’s place and finds Fernando sitting on the bed, and Shayla passed out in the bathtub. While Shayla asks Elliot not to involve the police due to the nature of her business and her reliance on Fernando as a supplier, Elliot tips off the authorities about Fernando and his gang, leading to their arrest.

Inside the jail, Fernando realizes that it must have been Elliot who had gotten him arrested, and thus he starts pressuring him to take him out and threatens him that if he does not do that, Shayla will be killed. Elliot eventually agrees, and by hacking the prison system, he helps Fernando escape.

After coming out of prison, Fernando asks his friend DJ to shoot his brother Issac, as it turns out that it was apparently Issac who had wanted to finish Fernando in prison, one of the reasons he was so keen on getting out at any cost. As Fernando starts walking away with DJ, he gives Elliot a key to a car, and upon opening the trunk of the car, Elliot finds Shayla’s lifeless body lying there.

It therefore becomes clear that Fernando had gotten Shayla killed beforehand and Issac had done the job. It was Fernando’s revenge on Elliot, and after this act, he vanishes completely, only to resurface during the post-credit scenes of the season 3 finale.
