Episode 1 opened with some high-stakes surgery happening at midnight, which was interrupted by an announcement of a strike. This takes us back to two months before this event, and we meet Biel de Felipe, a young and idealistic resident doctor joining the hospital. His first case brings him into confrontation with Patricia Segura, the president of Valencia. He discovers that she has breast cancer, and the oncologist, Dr. Nestor, warns him about the consequences. Meanwhile, Quique is dealing with a drug problem while also treating patients.

Then come episodes 3 and 4. These portray the three generations of doctors, such as Biel, who is an idealist, veterans like Nestor, and opportunists like Dr. Lluis Blasco, functioning and posing challenges for each other. Dr. Pilar Amaro also emerges as a brilliant but rigid surgeon whose son, Oscar, is secretly struggling with addiction.

Tension escalates when Oscar overdoses and is rushed to the hospital. After this, Pilar launches her own investigation, only to discover that the enabler is none other than her friend Quique. Meanwhile, Dr. Jesica is managing an overwhelming caseload while mentoring her younger brother, Rodri. Rodri is shown as someone more interested in online fame than social media. The season at this point covers several issues faced by medical staff, like budget cuts, working inhumane hours, and lack of supplies, setting up the foundation for major conflicts to unfold.

In episodes 5 and 6 of Breathless, tragedies start hitting the medical staff

Jesica orders Rodri to treat a young girl with breathing problems. Rodri, who lacks experience, misdiagnosed her as anxious, which further led to her worsening condition. This is when Jesica discovers she would need urgent surgery. Jesica is pulled to another emergency, and Pilar is barred from operating on anyone as Oscar is in the operating theater. It was Rodri who supervised the surgery himself without proper consent. As a result, the young girl dies, leaving Rodri shattered and, for the first time, realizing the extent of his actions.

The young girl's mother confronts him and says that she will never forget his mistake. This is when Rodri loses control and jumps off the hospital roof. He survives in a coma for a while but ultimately dies.

Rodri’s death becomes the spark that ignites collective outrage. The doctors and nurses realized how an inexperienced doctor had to perform a surgery because the staff wasn't sufficient. Their compassion and professionalism have been exploited, and this system is treating them as disposable. Nestor organizes a formal protest and strike where all the medical staff will stop work for 5 minutes at midnight. These 5 minutes resemble a gesture of unity and resistance but end up causing even more tension.

This brief halt compelled them to confront its consequences, posing a challenge to their idea of resistance and their inevitable responsibility.

This is how season one ends, following the flashback and ending right where it started.
