Previously, we saw how after Thony discovered Ramona and a Cowboy Hat assassin's involvement in Jorge's ex-wife Vanessa's death, Ramona was taken into custody. While Jorge was on a mission to track down the Cowboy Hat assassin. The synopsis of The Cleaning Lady Season 4 Episode 9 reads,

The Cleaning Lady Season 4 Episode 9 recap: Who put a bomb in the hospital—and why won't Thony escape?

In this episode, we see Thony back at the hospital after her recovery and Ramona's arrest. She comes across Maria, a patient with fresh stitches. Maria reveals that she is an illegal immigrant who works for a drug dealer. Thony soon finds out that Maria's life is starkly similar to her initial journey. Thony thus makes up her mind to save Maria at any cost. The test results show something shocking — a bomb has been planted inside Maria's body. Soon, Thony learns that the Cowboy Hat is behind the bomb- and that he's out for revenge.

Thony refuses to leave the patient alone, and Dr. DuPont accompanies her after Thony promises to reveal her secrets. They informed the bomb squad and began to operate without waiting for the combat surgeon. After an intense operation, DuPont, Thony, and the bomb squad are finally able to neutralize the ticking bomb.

As promised, Thony comes out clean to Dr. DuPont. She reveals everything - her involvement with the cartel and her fake marriage to Jorge. Dr. DuPont listens to her and, in an emotional moment, reveals that she cares for her deeply. The two share an intimate moment in the on-call room.

Since Jorge came to know about Ramona and Cowboy Hat's involvement in his wife's murder, his motive has been clear- to hunt down the Cowboy Hat assassin. He was too shocked to hear about his sister's involvement in Vanessa's death. After Ramona's arrest, he kept his daughter Violeta with Thony and Luca and set out on his mission. However, as a result, his business and cartel went for a toss. He cancels his meeting with Sam Heller and tracks down the Cowboy Hat. After an intense face-off, Jorge finally kills the Cowboy Hat. However, before dying, he reveals that Ramona had choked his wife, Vanessa, causing her death.

After the chaos, when Thony and Dr. DuPont were sharing an intimate moment, Thony received a voicemail on her phone. In the voicemail, Jorge confessed his feelings for Thony and how he is ready to build a family with her.
