While the premise is set in a Connecticut town, I Know This Much Is True was actually shot in the state of New York. The filming took place in 2019, with the following four towns, cities, and counties serving as the production locations:

1. Poughkeepsie, Dutchess County, New York

The show was primarily filmed in Poughkeepsie, the county seat of Dutchess County in New York. Several locations in the town are visible in the series, including a house on Delafield and Hoffman Streets, which is shown as the Birdsey family's home. The simple appearance of the house was ideal for providing a peek into the life of a working-class family.

Other locations in the town featured in the show include the Mid-Hudson Bridge and the Poughkeepsie Journal Building. The Mount Carmel neighborhood also served as a filming location for several scenes.

At the same time, the under-construction Vassar Brothers Medical Center was transformed into a fake casino, which appeared in the final episode. Vassar College, a private liberal arts college in Poughkeepsie, represented not just one, but two places in the show: the University of Connecticut and a humanities building.

2. Fishkill, Dutchess County, New York

Several scenes of I Know This Much Is True were shot in the town of Fishkill in Dutchess County. One prominent location that appears in the film is the Seasons Restaurant. The restaurant serves a dose of nostalgia to viewers as it has now been shut and replaced by a new food outlet called Casa.

The All Sport Health & Fitness Center, located at Old Main Street, Fishkill, was also utilized for some key sequences. The town was an ideal place to showcase the routine life of the people of Three Rivers.

3. Ulster County, New York

Another location in New York that hosted the cast and crew of the show was Ulster County. Route 209 in the county was shut down for filming scenes showing Dominick driving on the road. The flashback scene where the Birdsey grandfather throws a weighted sack into the river was filmed at the Wallkill Valley Rail Trail bridge in New Paltz.

Additionally, Pleasant Valley Estates was used as Dominick’s adult apartment, and the 9 Mall Plaza in Wappingers was turned into a Blockbuster Video store to depict the 1990s. Thomas’s hospital scenes were filmed at the Taconic Developmental Disabilities Services Office in Wassaic.

4. Newburgh, Orange County, New York

Since Mark Ruffalo played a dual role in I Know This Much Is True, the production team needed a studio to film scenes featuring both brothers. These sequences were shot at the Umbra Sound Stages in Newburgh. Located at 9 Scobie Drive, Newburgh, the studio made it easy to handle special effects and indoor scenes.
