Grey's Anatomy, ABC's longest-running medical procedural series, premiered its milestone 450th episode (Season 22 Episode 2) titled We Built This City on October 16. While it was certainly a celebratory moment for the show's veteran and new cast, the mood of the story is currently sombre.
The season premiered in the aftermath of the Grey-Sloan explosion, which took Monica's life and left several characters' fates hanging in the balance. Caterina Scorsone's Dr. Amelia Shepherd made a shocking decision in the latest episode and announced her sabbatical.
Amid the rapidly changing cast dynamics in ABC's medical procedural, Caterina Scorsone's Amelia felt like a constant and a nostalgic reminder of the good old days. Scorsone debuted the series back in Season 7, when she was introduced as the sister of Derek Shepherd (Patrick Dempsey). While Dempsey quit Grey's Anatomy a long time back, his character's sister serves as a quiet reminder of his legacy.
However, with Amelia Shepherd’s temporary exit from Grey’s Anatomy Season 22, fans will certainly miss this beloved character.

Grey's Anatomy: The truth behind Amelia's sabbatical
Grey's Anatomy fans were surprised when Grey-Sloan veteran Amelia Shepherd revealed that she is taking a sabbatical in the aftermath of the explosion in the Season 22 premiere, which resulted in her friend Dr. Monica Beltran's death.
While Amelia's condition and eventual decision after that brutal explosion were devastating, it was a result of the behind-the-scenes creative decisions at ABC. As Deadline previously reported, many networks are adopting different schedules for their 2025-26 procedural cast. Grey’s Anatomy has lowered the episode guarantees for its veteran cast, reducing their appearances to a minimum of 14 out of the season’s 18 episodes.
This was decided before the show returned for Season 22. While fans were prepared for long-tenured cast members and their beloved characters' absence in four episodes, Caterina Scorsone seems like an exception. According to a Deadline report, Scorsone will now rejoin Grey's Anatomy Season 22 after its mid-season finale (in 2026) and will appear in only 10 episodes of the latest season (out of 18).
While other cast absences will be peppered throughout the new season, Scorsone's Amelia will remain consecutively absent from episodes.
How was Amelia Shepherd's exit written in Grey’s Anatomy Season 22?
The second episode of Grey's Anatomy Season 22 began in the aftermath of the explosion. Meredith's sister Maggie flew in from Chicago to help her cheer Amelia, who was consumed by guilt and trauma from the explosion and her friend's death. Since the explosion was initiated by one of Amelia's patients' mothers, she blamed herself for the fiasco.
While Amelia was doing considerably better throughout the episode, she eventually announced her sabbatical from Grey Sloan at the end of the episode. The toll of the explosion, Monica's death and Dylan's condition were too hard for her to bear and she finally reveals that she will go “wherever my people are'' to Meredith and Maggie.
As per Deadline, Amelia's sabbatical will continue well into the Grey's Anatomy Season 22 mid-season finale and she will only return in 2026.
Grey's Anatomy milestone episode also revealed the fate of Amelia's patient, Dylan, who might remain locked in her body and spend the rest of her life in a care facility. In an interview with Deadline, showrunner Meg Marinis teased an emotional struggle for Amelia, which might materialize after she returns from her sabbatical. Marinis said:
“We’ll see if her strive and desire to keep taking on medical cases, we’ll see if that continues after what happens to Dylan. I think that’s a moment of hubris for her. She took on something that, there’s a possibility she shouldn’t have operated on that girl to begin with.”
