Showrunner Meg Marinis shared her excitement about the Season 22 premiere with TV Insider, revealing that it had been a while since the show had featured a life-threatening cliffhanger. Since the Season 21 finale could not explore the aftermath of the explosion, Only the Strong Survive will begin moments after the explosion.

This major catastrophe will rule the characters' lives in the premiere and they will need to find a balance between their professional duties and personal relationships.
