Showrunner Meg Marinis has been candid about the reason behind Monica Beltran’s death, speaking to Deadline.

When asked why they chose to kill Monica Beltran in Grey’s Anatomy Season 22, Marinis revealed:

“The decision on who we were going to kill off, we knew it was going to be one out of that group of people that was on the OR floor, but I really struggled with making the decision to kill someone off, I didn’t want to kill someone off. I left last season thinking, no, we’re not killing anyone off. But then I was in the writers room, and as I rewatched the finale, it became clear to me, Oh, this is where the story is leading.”

About giving her a quiet goodbye, Marinis told Deadline:

“I think it speaks a lot about who her character is and was; she’s very selfless and she’s always been about the children first, taking care of them, saving their lives, doing anything she can to protect them and make sure that they’re okay. And she’s also been an excellent teacher. There’s been a couple of moments — and she even references them — where she’s been with Jules when Jules has almost frozen in an OR, frozen on a case, and she wasn’t going to let that happen today. She was going to be there for Jules. I think it just says who she is as a teacher, that she dies still responding to the calling that she had in life, which was to be a pediatric surgeon.”

Grey’s Anatomy Season 22 premiered on ABC on October 9, 2025. As the curtain rises, you are right back in the aftermath of the OR explosion from the end of Season 21. There is panic, alarms blaring, people shouting, and nurses sprinting with the chaos of Grey Sloan.

Right away, you are just glued to the screen, counting heads, hoping your favorites aren’t dead. Turns out, most of the regulars get lucky and your precious main characters dust themselves off, shaken but intact. But then, we get to know that Link and Monica Beltran aren’t among the lucky ones.

Link is found unconscious, and there is a frantic, pulse-pounding rush where his colleagues desperately try to save him, and after some serious build-up, they manage to do it.

On the other hand, Monica’s story is brutal. She is pinned under surgical equipment in another OR. But Monica, even trapped and barely hanging on, is guiding Jules through the most stressful first solo surgery. Monica walks her through every step, all while you can see her fading.

When the kid patient survives, you get this fleeting hope that maybe Monica will make it out, too. But before anyone can get to her, Monica slips away. It’s not loud or showy; she just… goes. She saves a life while losing her own.

When the rescue team finally gets in, they find Monica already gone. The characters are wrecked, and the emotional punch really lands with Jules. She is absolutely shattered after telling the boy’s family he is okay, but knowing Monica didn’t make it. Winston steps in to comfort her, but you can tell nothing is really going to fix it.
