As mentioned, Milo is never seen when he visits his best friend's home to play. It is discovered that Carrie Jenny's nanny has also been missing since then, and this leads the authorities to chase her down. While going through CCTV footage, Carrie is seen arguing with her ex-boyfriend, Kyle Smith, at the Chicago Marathon. This makes Kyle a part of the plan, and as the investigation deepens, it becomes clear that Milo hasn't been abducted for money or ransom but because of a personal reason.

The finale episode reveals that Carrie is the biological mother of Milo, while Marissa and Peter had lost their son years ago in a brutal car accident. Marissa isn't aware of this, and Carrie had gradually come across Milo, realizing his motherly instinct for him. Her doubt cements when she finds Milo has the same rare genetic disease that can only be inherited from parents. Moreover, Milo's age also exactly matched the number of years the Irvine family suffered the car accident. This makes her furious about the suffering she has faced because of the family and their wrongdoings, and she decides to take back what belongs to her.

Years ago, when Milo was a newborn Irvine family suffered a car accident. However, they were not alone in the incident. It was Carrie and her newborn child who were in the other car. Peter, when he gained consciousness, realized that they had lost their child. This is when he acts coldly and swaps his child with Carrie's to escape years of despair and guilt.

As mentioned, the All Her Fault finale episode sums up the suspense the episodes have been building all this time. Marissa is informed that Milo is safe and alive. His existence exposes how love can become dangerous when twisted by guilt, grief, and control. Carrie leaves Milo, telling Marissa to take good care of him and keep him away from Peter. While the show also reflects on how Peter's decision to steal the baby was not purely evil, it stemmed from unbearable loss. However, his act of deceit has changed several lives and especially Milo's fate, around his identity.

Milo appears to be a symbol of innocence caught in adult corruption, becoming a child who is loved and protected but is also stolen and kept in the dark. The child lives, but the family he always knew has been destroyed.
