At the end of season one, the Fireflies (doctors) are hoping to create a vaccine using Ellie. But the procedure would kill her.
This is why Joel proceeds to save Ellie, kill the fireflies, and leave the hospital. Then, he proceeds to lie to Ellie that the fireflies were unable to design a cure. But in a flashback scene in The Last of Us Part II, Ellie returns to the hospital in Salt Lake City and uncovers a recording. This recording reveals the truth, and Joel reluctantly confirms it, straining their relationship.
Ellie might be feeling guilty after learning that Joel took away humanity's last chance to end the Cordyceps infection for good by saving her. Furthermore, Joel lied to Ellie by swearing on her even in the Season 1 finale that was aired in 2023. This is the reason why Ellie is mad at Joel in the Season 2 premiere. It is yet to be revealed how they resolve this issue before the show adapts the heartbreaking moment from The Last of Us Part II game.
