The series begins with maniacal trauma surgeon Kang-hyuk's entry in Hankuk National University Hospital. Initially, no one liked him as he was directly sent by Myeong-hui, minister in the Ministry of Health and Welfare. From his ways of working and treating his patients, to forcing Jae-won into accompanying him in his trauma surgeons, Kang-hyuk creates menace all around the hospital.

However, as the series progresses, people in the hospital warm up to him, with Jae-won now aiming to become a trauma surgeon, rather than joining general surgery. The K-drama also revealed Kang-hyuk's past about working with a mercenary group called Black Wing, in South Sudan.

At the hospital, he endures political schemes against him, claims of malpractice, and several other challenges created by the Directors of the Hospital, but amid all of those problems, Kang-hyuk gave priority to his patients and was ready to treat them in the Hospital ER or amid flying bullets in South Sudan.

Besides Ju Ji-hoon, the Kdrama also stars Choo Young-woo, Ha Young, Jung Jae-kwang, Yoon Kyung-ho, and Kim Sun-young, among several others. Furthermore, if the K-drama gets renewed, these actors might again be seen in The Trauma Code: Heroes on Call Season 2.
