Featuring South Korean A-list stars Lee Jong-suk and Lim Yoon-a, Big Mouth serves as a thrilling K-drama that centers around the life of Park Chang-ho, a struggling lawyer who barely wins cases and is always discarded by his superiors. Despite his willingness to prove himself, he ultimately fails.
On top of that, Chang-ho has a habit of talking too much without backup, due to which people mockingly call him "Big Mouth." This is the nickname everyone uses while discussing the lawyer, but things take an unexpected turn when a misunderstanding emerges.
Chang-ho finds himself in grave danger when he's wrongly identified as "Big Mouse," a highly skilled conman who terrifies the upper class. The worst part? The police throw Chang-ho behind bars as he ends up being accused of drug use, fraud, and more crimes.
As a result, Chang-ho was left with no option but to take up the identity of Big Mouse, which was forced onto him. From behind bars, Chang-ho starts digging out the truth and learns about a group named NR Forum, made up of the wealthiest and most corrupt elites.
However, Chang-ho isn't alone. He receives support from his wife, Ko Mi-ho, who happens to be a nurse and uses her intelligence during the investigation to help Chang-ho.

Big Mouth K-drama ending explained: Mi-ho's heartbreaking sacrifice

Mi-ho in Big Mouth is played by Yoona, who recently won hearts through her portrayal of Yeon Ji-young in Bon Appétit, Your Majesty K-drama alongside Lee Chae-min. Unlike her recent role, Mi-ho is quite an emotional wife, who desperately wants Chang-ho to get out of prison and prove his innocence.
So, to help Chang-ho, she starts an investigation and finds out about NK Chemical’s shady activities. However, the most heartbreaking moment comes when Mi-ho unknowingly gets exposed to radioactive wastewater and develops terminal lymphatic cancer.
Despite her worsening health conditions, Mi-ho refuses to give up. She digs out everything about the culprits and exposes them publicly. This way, everyone comes to know that NR is a ruthless and criminal group threatening people's lives.
In the final moments, Mi-ho was able to meet Chang-ho for the last time and died in his arms. Her departure fueled Chang-ho's determination to take down NR Forum and bring justice to his wife.

The corrupt mayor Choi Da-ho meets his end in the finale of Big Mouth K-drama
After Chang-ho witnessed his wife's sacrifice, he decided to take down the elite group, especially Mayor Cho Da-ho. He managed to escape prison and even silenced his wife, who had the evidence to prove that Da-ho is a criminal.
After getting his wife locked up in a mental hospital, Da-ho prepared to win the case in court. With no concrete evidence against him, he won the battle. But Chang-ho didn't give up. He manipulated Da-ho and lured him to the swimming pool.
This time, it was filled with the same deadly chemical that killed Mi-ho. As Da-ho went inside the pool, Chang-ho enjoyed his death. He watches Da-ho vomiting blood and experiencing a painful death just like Mi-ho. This way, Chang-ho gets his revenge and fully embraces the identity of Big Mouse to take down other corrupt figures.

After Choi Do-ha dies in the Big Mouth Kdrama, Park Chang-ho fully accepts being “Big Mouse,” the identity he was once falsely accused of. He takes charge of the power and name that used to cause him trouble and starts using it for a good purpose.
He manages to get hold of Seo Jae-young’s research paper, which turns out to be really important: it proves that a chemical called NF9 had been causing cancer in people for many years. He also somehow finds Do-ha’s hidden stash of gold, though the show never actually shows how he did it, which left viewers a bit puzzled.
Thanks to all the new evidence he uncovers, the three VIPs involved in the crimes are sent back to prison for murder. After that, Gong Ji-hoon becomes the new head of the Woojeong Group, taking over the powerful position Do-ha once had.
Meanwhile, donations start showing up at orphanages and nursing homes. They are all signed with the "Big Mouse" symbol. This is how Chang-ho starts using his new role to help people instead of hurting them.
Big Mouth ends on a meaningful note: Chang-ho drives through a tunnel lined with white flowers, suggesting his sadness over Mi-ho’s death and the change in him. By the end, he becomes the man that his late wife always wished for.
