Sandy returned to Salem with her mother in 1967 and immediately began to develop a relationship with her grandparents Tom and Alice Horton. In 1968 when Mark Brooks was revealed to be her father Tommy Jr. she and her father struck up a relationship. After her mother died Sandy resented the other Hortons for the way they treated her mother, and in 1970 Sandy moved into a dorm room at school. In 1982 Sandy Horton became a doctor at University Hospital, and later became engaged to Evan Whyland. However, when she learned Evan had been cheating on her she broke off the engagement and told Maggie she could kill Evan for what he did. Later Evan was admitted for emergency surgery, and Sandy had to perform it. When Alex Marshall switched Evan's medical records Sandy gave him the wrong penicillin, and as a result Evan died. Sandy was suspended, but was later reinstated when she was cleared of any wrong doing. In 1983 Marlena and Sandy Horton witnessed Roman Brady kill Daisy on the docks. They were shocked, and even though Marlena was married to Roman she reported the crime and Roman was arrested for the murders.
