Bella Ramsey got diagnosed with autism while they were filming for the first season of HBO’s The Last of Us. They revealed to British Vogue, “I got diagnosed with autism when I was filming season one of The Last of Us.”

As a self-described "loner" and "weirdo," Ramsey acknowledged that they "always wondered" if they had autism as a child. But they didn't do anything about it until a crew member who had a daughter with autism thought Ramsey had it. They sought a professional mental evaluation and diagnosis as a result. A brain development abnormality called autism spectrum disorder affects how a person perceives and communicates with other people, leading to issues with communication and social interaction. Limited and repetitive patterns of conduct are another aspect of the illness. The word "spectrum" describes the broad range of symptoms and severity.

Early childhood is the starting point for autism spectrum conditions, which later leads to issues with social, academic, and professional functioning. Children often show autism symptoms in their first year of life. According to Ramsey, it has benefited their acting career in the long run. “I’ve always been watching and learning from people. Having to learn more manually how to socialize and interact with the people around me has helped me with acting.” Ramsey claims to be "still figuring" out gender designations and stated that being officially diagnosed with autism and given a name felt "freeing."

In contrast, Ramsey told the news organization, that disclosing their autism diagnosis enables them to "unmask immediately." “My experience of moving through the world is as an autistic person,” they stated. “There’s no reason for people not to know.” They stated, “It enables me to walk through the world with more grace towards myself about not being able to do the easy everyday tasks that everyone else seems to be able to do". “The label of being autistic has been so helpful to me because that’s helped me to understand myself, but gender and sexuality-wise, labels do not feel comfy for me in any capacity, because I feel like I’m putting myself into a box. I feel trapped.”
