Sanada was found by an entertainer while playing with the actor Kokichi Takada's son, who shared a condominium with him. He was born in Tokyo. At the age of five, he began performing with the Himawari Theatre Group after modeling for a young children's magazine. He trained under Shorinji Kempo intending to become an action movie star, and he later started Kyokushin kaikan karate. He started as a catcher in baseball at the age of eight. Sanada joined Shinichi "Sonny" Chiba's Japan Action Club and received mentoring from the actor.

Sanada played the son of Chiba's character in the 1966 movie Game of Chance in his debut performance at the age of five. To concentrate on having a typical upbringing, he temporarily left his roles in Japanese television and movies. By the time he was a teenager, he was able to act, fight, sing, dance, and saddle a horse. Seeing that many famous actors would do so, Sanada thought it was "the best service for the audience" to learn these skills himself before he started performing again. He returned to acting with a part in a Kinji Fukasaku-directed Japanese movie.
