Tom Maynard was rushed to the Seattle Grace Hospital alongside several other victims of a rail tragedy. A metal pole had pierced both Tom and Bonnie Crasnoff's torsos, pinning them together. After an extensive exam, Seattle Grace's medical staff soon decided that to save the lives of Tom and Bonnie, one of them would need to be taken off the pole.

Doctors concluded that Bonnie needed to be removed from the pole because of her terrible injuries, which included a torn aorta and a damaged spinal column. Tom was hesitant when the doctors asked him and Bonnie about their choice because he thought it was unjust that they had chosen his life above hers. Bonnie eventually convinced Tom that since he had a higher chance of surviving, it was okay for her to be moved away.


Bonnie asked Tom if he believed in heaven when they first entered the operating room, and Tom reassured her that he did. Bonnie was taken off the pole and died shortly after Tom and Bonnie were anesthetized. Fortunately, Tom survived the entire incident, and his operation went well. Later, Tom was spotted with his appreciative and thrilled wife in a recovery room.

In light of the dire situation, Tom and Bonnie developed a strong emotional bond while they were stuck on the pole. Bonnie and Tom both made jokes about breath mints and pores to lighten the mood, despite the seriousness of their situation.
