How does Andrea die in The Walking Dead?

After being captured by The Governor, Andrea is retrained in his torture chair and locked in a prison cell. But as he is also unhappy with Milton, the Governor beats him up and then asks him to kill Andrea to live, but Milton refuses to do so. Instead, he attempts to kill the Governor himself, who stabs Milton.

He then locks the dying Milton with a restrained Andrea in the cell and leaves, which is when Milton tells Andrea that he left pliers behind her chair. She attempts to get to them before Milton dies and turns into a walker. She nearly manages to do that but it’s too late as Milton bites her just as she frees her hand. Andrea then kills Milton and sits waiting as Rick, Michonne and the rest enter the cell a little while later to find her.

She asks Rick for his revolver to commit suicide and Rick obliges, giving her the gun and stepping outside. Andrea then bids her farewells to her friends and everyone steps outside with only Michonne staying behind as the shot rings out off-screen.
