Billy Bob Thornton made a guest appearance on The Big Bang Theory season 8 episode 7 titled "The Misinterpretation Agitation." He played the character of Dr. Oliver Lorvis, a urologist. He misunderstands Penny’s actions as flirting and visits her apartment with flowers. Leonard clears his misunderstandings but gets to know that Penny doesn't wear her Engagement ring at work. Sheldon invites Dr. Lorvis to his apartment to have some hot beverages because he was upset.

Penny shows up and is confused to see Dr. Lorvis there. When Leonard confronts her, she explains that she doesn't wear the ring at work in order to boost sales. The boys are impressed by Dr. Lorvis and all of them visit his house to see his collection of action figures and movie props. While they are busy looking at his collection, he locks them in his basement and visits Penny again to try his luck but on the way he meets Amy and falls for her instead.

Penny realizes that he has locked the boys in his basement and makes him understand that he should not chase every girl who behaves nicely with him. Penny makes him let the boys out. On the other hand, the boys were having fun playing games and were not ready to get out of the basement.

The creators of the show saw one of the red carpet interviews of Billy Bob Thornton where he said that he loves to watch The Big Bang Theory and gets so into it that he starts talking to the characters of the show on the screen. They thought it would be great to invite him on the show sometime if they come up with something that suits him.

When they come up with the idea of Penny not wearing her ring at work, they thought that Billy could play the role of a socially awkward doctor who misinterprets Penny’s actions as flirting. They went to Billy and his team with this idea, and Billy liked the idea and agreed to be part of the show.
